We are always accepting resumes for qualified Transitional Care Nurses for a local hospital system in the Louisville, Ky area. Most contracts range from 8-13 weeks & travelers are encouraged to apply. The ability to provide outstanding patient care, multitask, and take direction is a must for this position along with 2 current years hospital experience.
Responsibilities:
- Performs a comprehensive physical assessment utilizing observation techniques and all available data including lab results, procedure/test results, physician findings and findings of other disciplines.
- Establishes nursing plan of care, utilizes other disciplines in the establishment of plan and determines appropriate nursing interventions in accordance with patient needs and nursing standards.
- Documents care given by recording assessments, nursing care, observations and patient’s progress.
- Instructs patients and family members regarding treatments, surgical procedures, nursing plans and home care to facilitate successful post-discharge care and patient understanding. Conveys, shares and documents advanced knowledge base.
- Follows guidelines that maintain safety for oneself, patients, visitors and co-workers and if classified as a category I or II employee under OSHA regulations, adheres to OSHA standards by wearing personal protective equipment, following universal precautions and being knowledgeable about blood borne pathogens.
